By consenting to join the Master Godifestors' Playground, you are agreeing to the following terms:

Confidentiality: We are not HIPAA compliant; however, the personal information discussed in the Master Godifestors' Playground will absolutely not be disclosed outside of the group unless you give us permission, are a danger to yourself or others, or you disclose knowledge about abuse of a child or vulnerable adult. You also agree not to disclose other members’ information outside of the group. Please note, we cannot guarantee the full security of information discussed via Zoom or Voxer or by other participants but all members of the group agree to this agreement prior to participating in the Master Godifestors' Playground and any violations of confidentiality result in removal from the group immediately without a refund.

Informed Consent: There are potential risks of participating in the Master Godifestors' Playground. Given the personal nature of our discussions, there may be risk of increased stress and discomfort. We are here to support you during this process and encourage open communication about your needs.

Rates: The rate for the Master Godifestors' Playground is $1111 USD paid in full or two monthly payments of $600 USD. For payment plans, an initial payment of $600 USD must be paid prior to enrolling. The subsequent payment is renewed automatically using the same payment method on the same day of the following month as your initial payment. If the subsequent payment is unsuccessful, you have two business days to resolve payment, and will otherwise be removed from the group and no longer have access to any of the recordings. All fees are non-refundable even if you choose to leave the Master Godifestors' Playground. To discontinue participation at any time, inform us in writing by sending an e-mail to [email protected].

We do not diagnose, treat, or medicate any medical or psychological conditions. The purpose of this Master Godifestors' Playground is to provide support and encouragement. This Master Godifestors' Playground is not a replacement for professional psychological and/or medical treatments. We invite you to consult your medical/psychological professionals to make an informed and educated decision about joining the Mastermind. Using Aloha Center for Transformation & Wellness, LLC dba Return to Aloha’s/Candace Tucciarone’s/Brian Tucciarone's services is at your own risk without warranty of any kind. The services are requested by your own choice and with your inherent singular responsibility. Aloha Center for Transformation & Wellness, LLC dba Return to Aloha/Candace Tucciarone/Brian Tucciarone shall not be held liable for claims or damages, and liability of any nature, for any action, or non-action, taken as a result of the Master Godifestors' Playground. I hereby acknowledge that no guarantees have been made to me as to the effect of the Master Godifestors' Playground. I have read this form and the requirements of the group and certify that I understand and agree to the contents and I am considered a legal adult who can consent in my name. I understand that if I violate the confidentiality or am disrespectful to other members, I will be dismissed from the group immediately without a refund.
