By consenting to enter a coaching relationship with a coach from Aloha Center for Transformation & Wellness, LLC dba Return to Aloha, you agree to the following terms:

Confidentiality: We are not HIPAA compliant; however, the information discussed in your coaching relationship will not be shared unless you give us permission to include you in our promotional materials, are a danger to yourself or others, you disclose knowledge about abuse of a child or vulnerable adult, your records are subpoenaed by a court of law, or you require medical care, in which we will provide relevant information about you to those involved in accessing and providing your care. Please note, we cannot guarantee the security of sessions that are recorded by Zoom, discussions via the Voxer app, and sessions uploaded to third-party platforms such as Kajabi.

Informed Consent: There are potential risks of this coaching relationship. There may be risk of increased stress and discomfort. We encourage open communication about your needs.

Rates: This coaching package is $100,000.00 USD. Fees must be paid at the time of booking. Fees are non-refundable and non-transferable. Coaching sessions and VIP Days must be completed within one year of enrolling in the program. Sessions can be rescheduled as long as 24 hour notice is provided. Sessions and VIP Days not completed within a year are forfeited. VIP Days include accommodations, airport transfer, three meals per day, and excursions (airfare is not included).

We do not diagnose, treat, or medicate any medical or psychological conditions. The purpose of this coaching relationship is to provide support and encouragement. Our coaches are not licensed medical or psychological professionals. This relationship, therefore, is not a replacement for professional psychological and/or medical treatments. We invite you to consult your medical/psychological professionals to make an informed and educated decision about entering this coaching relationship. Using Aloha Center for Transformation and Wellness, LLC dba Return to Aloha’s/Candace Tucciarone’s services is at your own risk without warranty of any kind. The services are requested by your own choice and with your inherent singular responsibility. Aloha Center for Transformation and Wellness, LLC dba Return to Aloha/Candace Tucciarone/Brian Tucciarone shall not be held liable for claims or damages, and liability of any nature, for any action, or non-action, taken as a result of this coaching relationship. This Agreement shall be governed by and construed in accordance with the laws of the State of Hawaii without giving effect to any choice or conflict of law provision or rule.

EMERGENCIES
If you are feeling suicidal, homicidal, or at risk of hurting yourself or others, you are agreeing that you will 1) go to your Local Hospital Emergency Room, or 2) call 911. You understand that Candace Tucciarone, Brian Tucciarone, and Aloha Center for Transformation and Wellness, LLC dba Return to Aloha do not provide crisis intervention or crisis services.

You hereby acknowledge that no guarantees have been made to you as to the effect of this coaching relationship or program. You have read this form and certify that you understand and agree to its contents and you are considered a legal adult who can consent in your name.
